Six-Pack Liposuction (Abdominal Etching) in NYC
By Dr. Itay Wiser, MD, PhD, Associate Clinical Professor of Surgery at Mount Sinai Elmhurst Hospital, fellowship-trained in aesthetic and reconstructive breast surgery at Lenox Hill Hospital. Medically reviewed October 2026.
Six-pack liposuction, also called abdominal etching or hi-def liposuction, removes fat selectively along the lines of the abdominal muscles so their outline shows. It is a definition procedure for men who are already lean and train regularly but cannot bring out the last layer, not a weight-loss procedure. Dr. Itay Wiser performs abdominal etching in NYC with VASER ultrasound liposuction and judges candidacy strictly, because the result looks natural only on the right patient.

Who is a candidate for abdominal etching
- Already lean, with a body-fat level low enough that the muscles are close to the surface.
- Trains regularly, so there is muscle to reveal.
- Stable weight for several months.
- Elastic skin that will contract smoothly over the new contour.
- A small, stubborn layer of fat over the abdomen that training has not removed.
Men who carry significant abdominal fat, or whose skin is loose after weight change, are not candidates for etching alone: standard liposuction or skin removal comes first, and Dr. Wiser says so at the consultation. The ab etching candidacy article walks through the self-assessment.
Etching versus standard liposuction
Standard liposuction removes fat evenly to slim an area. Etching removes a thin layer over the muscle bellies and leaves slightly more along the natural grooves, so the outline of the muscles shows. It is finer, slower work, and it needs the precision of VASER liposuction, which loosens fat with ultrasound before it is removed and allows careful sculpting with less bruising.
What to expect from the result
In the right patient the definition follows your own anatomy and looks like the product of training, which it partly is. Two honest limits: etching does not build muscle, so a patient without the underlying muscle will not get a six-pack from it; and weight gain afterward fills the grooves back in. Dr. Wiser shows before-and-after cases of men with similar starting points so expectations are set with photographs.
Recovery and when results show
Abdominal etching is outpatient, under local anesthesia with sedation or general anesthesia by extent. Expect a compression garment for several weeks, desk work within a few days to a week, and exercise when cleared. Swelling hides the definition at first; the result emerges over the following months as it resolves. The practice offers hyperbaric oxygen therapy to support healing. Timelines vary depending on the patient, the procedure and individual healing; your surgeon sets yours.
Cost
Abdominal etching is cosmetic and self-pay, priced by the extent of sculpting and the anesthesia used. The practice gives a written, all-inclusive estimate at the consultation and offers financing.

Questions men ask about abdominal etching
What is abdominal etching?
Liposuction performed selectively along the natural lines of the abdominal muscles, removing a thin layer of fat over the muscle bellies and leaving slightly more in the grooves, so the outline of the muscles shows. VASER ultrasound liposuction is used for the precision this requires.
Who is a candidate for ab etching?
Men who are already lean and train regularly, with a stable weight, elastic skin and a small layer of stubborn fat that hides definition. Men carrying significant abdominal fat or loose skin are not candidates for etching alone; standard liposuction, or skin removal, comes first. The candidacy article explains how to tell.
Does the result look natural?
In the right patient, yes: the definition follows your own muscle lines. In a patient without the underlying muscle, or who later gains weight, etched lines can look unnatural, which is why candidacy is judged strictly.
How long is recovery?
Compression garment for several weeks, desk work within a few days to a week, exercise when cleared, and the final definition visible as swelling resolves over the following months. Timelines vary depending on the patient, the procedure and individual healing; your surgeon sets yours.
Will the result last?
The removed fat does not return, and the definition lasts as long as your weight and training do. Significant weight gain fills the grooves back in.
