Explant With or Without a Lift: What Will My Breasts Look Like?

The fear is simple: that after implant removal your breasts will be empty, saggy or strange. The honest answer is that the result depends on four things, skin elasticity, implant size, years implanted, and how much the breast had already dropped, and that each combination has a predictable outcome and a predictable fix. Small implants in good skin usually settle into a natural, smaller breast on their own. Large, long-standing implants usually leave loose skin that a lift corrects in the same operation. This article shows you how to tell which you are.

The four factors that decide the result

  1. Skin elasticity. Younger, unstretched skin retracts after the implant is gone; skin that has been stretched for many years, or by pregnancy or weight change, does not fully shrink back.
  2. Implant size. The larger the implant, the more skin was stretched to hold it and the more is left behind.
  3. Years implanted. Decades of pressure thin the natural breast tissue and stretch the skin more than a few years do.
  4. Ptosis before surgery. If the nipple already sits at or below the breast fold with the implant in, it will sit lower without it.

Removal alone or removal with a lift: the decision table

Skin qualityImplant sizeNipple positionUsual recommendation
Good elasticity, few stretch marksSmall to moderate (under about 350 cc)Above the foldRemoval alone; let the breast settle; revisit in six months if wanted
Good elasticityLargeAbove the foldRemoval alone or staged lift; often settles better than expected
Fair elasticity, some laxityAnyAt the foldRemoval with lift, or staged lift after settling; decided by preference
Poor elasticity, stretch marks, thin tissueAnyAt or below the foldRemoval with lift in the same operation
AnyAnyAny, and you want some volume back without an implantRemoval with lift plus fat transfer

A planning guide, not a promise. Dr. Wiser examines your skin and measures your nipple position at the consultation.

How the breast settles over time

The first look after surgery is not the result. Swelling exaggerates fullness in the first weeks; as it resolves and the skin begins to retract, the breast changes shape through the first three months. The result is judged at three to six months, and some skin tightening continues for up to a year. That timeline is why staging a lift is a legitimate choice when the need is borderline: you see what your own tissue does before deciding.

With a lift: what changes

A lift removes the excess skin, raises the nipple to the centre of the breast and reshapes the remaining tissue so the breast sits higher and firmer without an implant. Some patients with enough remaining tissue are candidates for a lift that repositions their own tissue upward to rebuild upper fullness. Fat transfer can add roughly half a cup to a cup. The trade is lift scars, around the areola and usually a vertical line, which fade over the first year. The breast lift in NYC page explains the techniques, and fat transfer breast augmentation explains the volume option.

Seeing both outcomes

The most useful thing you can do before deciding is look at healed results of both kinds: removal alone, and removal with lift, in women whose starting point resembles yours. Dr. Wiser's before-and-after gallery includes removal without replacement, and at the consultation he shows you cases matched to your skin, implant size and nipple position. The breast implant removal (explant) in NYC page covers the full procedure, options, recovery and insurance.

Frequently asked questions

Will my breasts be saggy after implant removal?

It depends on four things: skin elasticity, implant size, how long the implants were in, and how much the breast had already dropped. Small implants in good skin often settle into a natural, smaller breast. Large or long-standing implants usually leave loose skin and a low nipple, which a lift corrects.

How long until my breasts settle after explant?

Most of the change happens in the first three months as swelling resolves and skin retracts; the final result is judged at three to six months. Skin keeps tightening slowly for up to a year in some women, which is why some surgeons prefer to stage a lift.

Should I have the lift at the same time or later?

At the same time when the need is clear: large implants, poor skin, a nipple at or below the fold. Later, after the breast has settled, when the need is borderline and you would rather see what your tissue does first. Dr. Wiser recommends one or the other based on your examination.

Can fat transfer replace the volume I lose?

Modestly. Fat transfer adds roughly half a cup to a cup using your own fat, with no new implant, and pairs well with a lift. It does not replicate a large implant.

Do the lift scars show?

A lift adds a scar around the areola and usually a vertical line to the fold, sometimes a line along the fold. They are hidden by a bra or swimsuit and fade over the first year and beyond.

In my experience, patients who have a clear understanding of what to expect and who actively participate in the decision-making process are the most satisfied with their results. For example, I worked with a client in April 2025 who was anxious about potential sagging after breast implant removal. We discussed all her options, including a breast lift and fat grafting, and she decided to proceed with a breast lift at the same time as her implant removal. After 3 weeks of recovery, she was thrilled with the results. She told me it was an amazing transformation that ultimately restored her self-confidence and gave her a more natural look. Her overall satisfaction score after the procedure was 9/10.
